Request Form 1095-C

There are 3 options to request a copy of your Form 1095-C for 2025.  Your form will be provided within 30 days of request:

1. Complete the Google Form here 1095-C Request
2. Email payroll@ouboces.org
3. Mail a request to OU BOCES Payroll 53 Gibson Road Goshen, NY 10924
